The COVID-19 pandemic has significantly impacted the social, economic, and cultural structures of communities, particularly vulnerable groups with limited access to resources. To address these challenges, community-based social transformation is a crucial strategy for rebuilding social resilience and strengthening citizen participation. This article aims to analyze the implementation of a post-pandemic community empowerment program through a case study approach in a local community that implemented a collaborative model between the government, civil society organizations, and residents. The research method used a qualitative approach with data collection techniques including in-depth interviews, participant observation, and program document analysis. The results show that the post-pandemic empowerment program was able to increase social solidarity, strengthen residents' economic capacity through entrepreneurship training, and expand access to health and education services. Key success factors lie in active community involvement in every stage of the program, transparent management, and support from external partner networks. However, challenges remain, such as limited funding, unequal participation, and resistance to change among some community groups. This study emphasizes that community-based social transformation is not only a recovery effort but also a long-term strategy for creating an inclusive, adaptive, and competitive society. The implications of this research emphasize the importance of multi-stakeholder synergy, strengthening local capacity, and program sustainability as a foundation for addressing future social crises. Thus, post-pandemic community empowerment can serve as a model for replication in other regions facing similar circumstances to promote more resilient and sustainable social development. Ultimately, post-pandemic community empowerment demonstrates the potential to transform crisis into an opportunity for systemic change. By fostering solidarity, enhancing economic resilience, and improving access to essential services, community-based approaches contribute to creating more resilient, equitable, and sustainable social development models.
